We have registered nurses, licensed practical (vocational) nurses, as well as certified nurse assistants to ensure that our resident’s skilled nursing needs are met. After sustaining an injury, rehabilitation is a key to recovery. Our team of skilled therapists and clinicians work closely with our resident’s orthopedic surgeon and/or primary care physician to design the best plan to work toward achieving their prior level of function.
Therapists work with patients in several areas: restoring strength, range of motion, rebuilding skills, and resume activities of daily living. Our gym is equipped with the latest equipment, a fully functional kitchen, and caring staff that provide hands on training, preparing and teaching each patient the skills they need in preparation for the return home.
At Oakpark, we are proud to provide outstanding therapy services including Physical Therapy, Occupational Therapy, and Speech Therapy. By employing an experienced, motivated therapy team that specializes in treating patients with health-related conditions, illnesses, or injuries that limit their abilities to move and perform functional activities, we are able to discharge the majority of our patients home or to a lower level of care after completing our rehabilitation programs. It is the goal of our therapy team to restore a patient’s ability to perform activities necessary for daily living and to obtain their prior level of function before an illness or injury when possible.
Our therapists establish a care plan, specifically tailored to meet the goals of each individual. The care plan may include pain management, exercises, stretching, manual therapy, and education, as well as other interventions. While all therapists work under the supervision of a Director of Rehabilitation, the Director ensures the recovery of our patients is being supported by all individuals involved with a patient’s care by working directly with our nursing staff and physicians.
Physical therapists work with individuals by maintaining, restoring movement and functional ability back to the patient, and preventing further loss of mobility before it occurs. Physical therapy is meant to help each individual build strength, endurance, coordination, balance, and/or flexibility with an ultimate goal is to restore maximal functional independence to each individual.
Our occupational therapists work hand-in-hand with physicians and surgeons to ensure that our patients reach their highest level of independence. Occupational therapists work with patients who suffer from physically, mentally, and/or emotionally disabling conditions by developing an individualized care plan that improves basic motor functions and reasoning abilities. Our occupational therapists provide customized interventions to improve individuals ability by providing treatment programs that may include range of motion, and strengthening exercises, functional ergonomics, and/or use of modalities to decrease pain and swelling.
Speech and language therapy is aimed at improving communicative abilities and cognitive functioning. The Speech-language Pathologists at Oakpark Healthcare Center tailor plans for patients requiring speech-language therapy. Our therapists workwith patients that have communication disorders and provide treatment in the areas of speech, voice, fluency, and swallowing.
Oakpark Healthcare Center is proud to offer extensive wound care by our team of board-certified wound surgeons. Our wound care team specializes in the treatment of various types of wounds including: pressure, diabetic, vascular, traumatic, and post-operative. Our wound care team is also capable of replacing gastric and suprapubic tubes in order to further reduce unnecessary transfers. Our specialized wound care nurse works directly with specialists and our nursing staff to organize and conduct weekly wound care rounds for patient consultation and follow up to ensure that wound are properly cared for and promote maximum healing potential.
At Oakpark Healthcare Center, we take pride in the social and psychological well being of each resident. Our program offers a wide range of activities designed to accommodate all patients at different mental and physical levels and abilities. By keeping our residents engaged through physical and mental activities active and in differ the interests of each resident through physical and mental activities.
Our recreational team offers a variety of activities: social, intellectual, exercise classes, spiritual programs, entertainment groups, and arts/ crafts classes are just some of the examples of activities our facility has provided. Our calendar changes monthly with new activities added on, we strive to provide interactive and fun activities, where everyone can participate. Activity and nursing staff motivate each resident by reminding, encouraging, and escorting our beloved residents to group activities of their choice.
At Oakpark Healthcare Center, we strive to serve our residents with appetizing and nutritious meals. Each resident, upon arrival, is assessed by our dietician to ensure that all the food preferences are met. For patients with diet restrictions, therapeutic menus are available. Our menus change with seasons providing a variety of assorted meals (meal substitutions are available upon request). In between meal times, our kitchen staff provides wholesome and nourishing snacks to our residents. At Oakpark Healthcare Center, we offer fine dining experience to our residents and families; you can enjoy your meals in one of our dining rooms, Cascade or the Garden Room.
At Oakpark, our Social Services Department is an integral part of ensuring the well being of our residents. Our social worker seeks to improve the quality of life for each resident by helping them, as well as their families, cope with emotional, social and medical needs. In addition to providing counseling services to our residents and their family members, our social worker also addresses grievances on behalf of the resident, and facilitates care conferences and support groups.
When a patient is ready for discharge, our social services director will work closely with residents and their families to provide the smoothest transition possible. While our facility is committed to providing the safest transition, we will coordinate any services needed after the resident’s stay at Oakpark such as transportation, in-home care, or providing adaptive equipment to ensure that each of our residents has a safe transition back home.
Beautician services at Oakpark Healthcare Center are offered every Wednesday. Residents can enjoy hair care services of a salon and make specific appointments with a licensed beautician based on their requests. If payment is not made on day of service, the resident will be individually billed on the 1st of following month. Appointments may be scheduled by our receptionist and payment may be arranged with the business office.
