At Oakpark, our Social Services Department is an integral part of ensuring the well-being of our residents. Our social worker seeks to improve the quality of life for each resident by helping them, as well as their families, cope with emotional, social and medical needs.
In addition to providing counseling services to our residents and their family members, our social worker also addresses grievances on behalf of the resident, and facilitates care conferences and support groups.
When a patient is ready for discharge, our social services director will work closely with residents and their families to provide the smoothest transition possible. While our facility is committed to providing the safest transition, we will coordinate any services needed after the resident’s stay at Oakpark such as transportation, in-home care, or providing adaptive equipment to ensure that each of our residents has a safe transition back home.
